All land activity, especially with prescribed fire, has risks for participants, owners, and organizers. To protect you, us, and others from liability claims and lawsuits, and to advise you about risks you may face, we require all participants of a PBA-assisted landowner burn, or any activity we organize, to fill out a liability waiver form and have it on file with us prior to participation. To participate, please initial each and every paragraph box, have your signature notarized, and mail the completed form to the mailing address shown on the form, or hand it in to a NWFPBA burn boss.
